Informed Consent

By booking and attending a permanent makeup appointment at D'Atelier PMU, you acknowledge that you have read, understood, and agree to the following:

Nature of the Procedure

Permanent makeup (cosmetic tattooing) involves the implantation of pigment into the dermal layer of the skin. Results are intended to be long-lasting but not permanent. Color will fade over time and touch-ups may be needed every 1-3 years.

Risks & Complications

As with any cosmetic procedure, risks include infection, allergic reaction, asymmetry, color changes, and unsatisfactory results. We take every precaution to minimize risks, including the use of sterile, single-use equipment.

Medical Contraindications

You must disclose all medications, medical conditions, and known allergies before your appointment. Services cannot be performed during pregnancy or breastfeeding, or on clients with certain autoimmune conditions, blood disorders, or active skin conditions in the treatment area.

Color & Healing Disclaimer

Pigments will appear darker and bolder immediately after the procedure and will soften by 30-50% during the healing process. Final results are not visible until healing is complete (4-6 weeks). Individual healing varies significantly.

Touch-Up Requirement

Most clients require at least one touch-up appointment 6-8 weeks after the initial procedure for optimal results. Touch-up pricing is separate from the initial service.

Photo Release

Unless you specify otherwise, photos may be taken before and after the procedure for portfolio and educational purposes. Your identity will be protected.
